Rule 205.2(b).    Cover Sheet.

(1)     Initial Pleadings.  A civil cover sheet shall be attached to any document commencing an action.  The civil cover sheet required shall be as follows:

(a)  Civil Cover Sheet.

(b)  Sanctions.  The failure to file the cover sheet as directed in paragraph (1) may result in an Order imposing sanctions.

(2)     Petitions or Motions.  The cover sheet required by Rules 208.3(b), 1028(c), 1034(a) and 1035.2(a) shall be as follows:
